Job Description

The Post Hospital Discharge Scheduler is the contact person for the inpatient case managers/coordinators to assist in scheduling all PCP and specialist appointments for qualified patients. Additionally, the Scheduler will contact patients to schedule appointments and remind them of their upcoming appointments. The Scheduler must be very organized, efficient, and professional, and prompt when interacting with other departments and patients. The Scheduler is also detailed in obtaining medical records and tracking hospital discharges and appointments.


SPECIFICS

1. Thoroughly, accurately, and timely track hospital discharges to determine which patients are required to have appointments at the Post Hospital Discharge Clinics.
2. Interact with inpatient case managers and coordinators to assist in making appointments for qualified patients with all PCP & specialists post-discharge.
3. Work with the inpatient coordinator to help make reminder phone calls the day prior to appointments.
4. Review internal systems for medical records and upload them to the Electronic Medical Record to be accessed by the clinician during the appointment.
5. Assist in rescheduling appointments for patients who cancel or don't show up for their scheduled appointments.
6. Maintain excellent tracking of the project, process, and scheduling.
7. Be proficient in Excel, as all of the tracking is done in Excel.
8. Filing, faxing, copying, and scanning of files.
9. Strive to meet personal, departmental, and organizational goals and targets.
10. Ability to manage a heavy workload with frequent interruptions and changing priorities in a fast-paced environment.
11. Completes assignments within specified time lines.
12. Able to work well in a team environment on special projects.
13. Skill in working independently, but with supervision.
14. Prepares and maintains records and files that are organized, thorough, and readily accessible.
15. Attend regular team meetings and report findings of projects.
16. When a problem occurs, inform the supervisor promptly so the problem can be corrected.
17. Transmits ideas and information clearly and concisely.
18. Assist the supervisor with special duties/projects.
19. Other duties as assigned by management.

The pay range for this position at commencement of employment is expected to be between $21/hour -$22/hour; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.


The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

• High school graduate required; Associate in Arts degree preferred.
• 2+ years of Healthcare or Managed Care experience
• Typing 60 words per minute with accuracy.
• Knowledge of computers, faxes, printers, and all other equipment.
• Proficient in MS Office programs (i.e., Word, Excel, Outlook, Access, and PowerPoint).
• Ability to deal with responsibility in confidential matters.
• Ability to work in a multi-task, high-stress environment.
