Job Summary: This Transplant Coordinator position provides primary clinical management and coordination of care for all post-transplant adult kidney, pancreas, and liver transplant patients. The Transplant Coordinator interacts with all necessary disciplines to coordinate patient services across the continuum of care. The person in this position must have exemplary customer service skills. Other duties as assigned.
Minimum of 3 years of acute care or dialysis experience is required. Experience relevant to Nephrology/Transplant sub-specialties is preferred.
Associate's degree in Nursing with a minimum of 10 years of relevant experience is required or Bachelor's degree in Nursing with a minimum of 3 years of relevant experience is required. Bachelor's degree in Nursing is preferred.
Analytical skills necessary in order to prepare and implement effective nursing care and develop solutions to problems concerning patient's mental or physical well-being. Ability to concentrate and pay close attention to detail for up to eighty percent of work time while: planning and performing professional nursing care, resolving patient care problems and dealing with patients, their families, and other professionals involved in the patients care, often during stressful situations.
Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) is required. Licensure Preferred: Certified Clinical Transplant Coordinator (CCTC) certification, Certified Clinical Transplant Nurse (CCTN), or Acute/Critical Care Nurse (CCRN) is preferred.
